Caring Hearts Medical Staffing (“CHM,” “we,” “our,” or “us”) respects the

understanding how we collect, use, and safeguard the information you provide
to us in using our websites and our mobile applications (collectively, “Sites”)
and the services provided through our Sites (collectively, “Services”).

1. Information We Collect Online
We collect Personally Identifiable Information from you through your use of
the Sites and Services. “Personally Identifiable Information” is individually
identifiable information about an individual consumer that we collect online
and that we maintain in an accessible form. We collect the following types of

Information You Provide
We may collect the following Personally Identifiable Information that you
voluntarily provide to us in using our Sites and Services:
• Apply. If you apply for a job, we will collect your name, email address,
telephone number, city, and state. We also will collect professional and
educational information such as your education history and professional
accreditation records, references, test results (e.g., medical, fitness,
psychological and drug/alcohol), financial information (to pay you), and
information contained in your resume/CV. You may choose to provide us
with your gender, race, veteran status, and disability status.
• Join Our Talent Network. If you join our talent network, we will collect
your name, email address, telephone number, areas of interest, city,
state, and preferred work setting.
• Job Alerts. If you subscribe to receive job alerts, we will collect your
email address.
• CTM Timeclock. If you use CTM Timeclock we will collect your username
and password. To the extent applicable, we also will collect other
information such as your timecard information.
• Contact Us. If you use our contact us webform, we will collect your
name, email address, telephone number, company name, zip code, and
any additional information you choose to provide.
Information as You Navigate Our Sites
We automatically collect certain Personally Identifiable Information through
your use of the Sites and Services, such as the following:
• Usage Information. For example, the pages on the Sites you access, the
frequency of access, and what you click on while on the Sites.
• Device Information. For example, hardware model, operating system,
application version number, and browser.
• Mobile Device Information. Aggregated information about whether the
Site is accessed via a mobile device or tablet, the device type, and the
carrier.
• Location Information. Location information from Site visitors on a city-
regional basis.
Third-Party Information
In some cases, we may receive certain Personally Identifiable Information from
you about a third party. For example, through our talent network service, you
may provide the name, email address, and phone number of another
individual. If you submit any Personally Identifiable Information about another
individual to us, you are responsible for making sure you have the authority to
do so and to allow us to use their Personally Identifiable Information in
accordance with this Privacy Policy.
2. How We Use Your Information
We use the Personally Identifiable Information we collect to provide the
Services to you, to improve our Sites and Services, and to protect our legal
rights. In addition, we may use the Personally Identifiable Information we
collect to:
• Process your account registration;
• Process your time though ADP services and CTM services;
• Process and fulfill your requests to be notified about job opportunities;
• Review your job application;
• Contact you regarding our products and services that we feel may be of
interest to you;
• Communicate with you about our Sites or Services or to inform you of
any changes to our Sites or Services;
• Provide support as requested;
• Maintain and improve our Sites and Services;
• Protect the security and integrity of our Sites and Services;
• Investigate, prevent, or take action regarding illegal activities, suspected
fraud, situations involving potential threats to the physical safety of any
person, or violations of our Terms of Use, and to otherwise fulfill our
legal obligations;
• Monitor compliance with and enforce this Privacy Policy and any other
applicable agreements and policies;

4. Cookies and Other Tracking Technologies
Like many other companies, we use cookies and other tracking technologies
(such as pixels and web beacons).
5. “Do Not Track” Signals
Some internet browsers incorporate a “Do Not Track” feature that signals to
websites you visit that you do not want to have your online activity tracked.
Given that there is not a uniform way that browsers communicate the “Do Not
Track” signal, the Sites does not currently interpret, respond to or alter its
practices when it receives “Do Not Track” signals.

Identifiable Information, we cannot guarantee absolute security.
8. Third-Party Links
The Sites and Services may contain links that will let you leave the Sites and
Services and access another website. Linked websites are not under our
control. Except as stated below, this Privacy Policy applies solely to Personally
Identifiable Information that is acquired by us on this Sites and Services. We
accept no responsibility or liability for these other websites.
9. Children’s Privacy
The Sites and Services are not intended for children under 13 years of age. We
do not knowingly collect, use, or disclose personal information from children
under 13.
10. Notice to Nevada Residents
Nevada law allows Nevada residents to opt-out of the sale of certain types of
personal information. Subject to several exceptions, Nevada law defines “sale”
to mean the exchange of certain types of personal information for monetary
consideration to another person. We do not currently sell personal information
as defined in the Nevada law. However, if you are a Nevada resident, you still
may submit a verified request to opt-out of sales and we will record your

11. NOTICE AT COLLECTION FOR CALIFORNIA EMPLOYEES
AND CANDIDATES
Job candidates and employees who reside in California may have rights
regarding their “Personal Information” as defined by the California Consumer
Privacy Act of 2018 (amended by the California Privacy Rights Act of 2020
(Collectively “CCPA”). This Notice describes how Caring Hearts Medical
Staffing. (“CHM”) collects, uses, shares, and retains your Personal Information
as part of its business operations, including when you apply for a job, accept an
offer of employment, or are engaged as an independent contractor on behalf
of CHM, or provide services on behalf of CHM to CHM’s customers or clients.
COLLECTION OF PERSONAL INFORMATION

We collect Personal Information directly from California residents and from
internet service providers, data analytics providers, government entities,
operating systems and platforms, social networks. We do not collect all
categories of Personal Information from each source.
WHY WE COLLECT PERSONAL INFORMATION
Our business purposes for collecting and using your Personal Information
include human resources, employment, benefits administration, health and
safety, and compliance. Many of these business purposes require that we
disclose your Personal Information to third parties such as clients and service
providers.
For example, CHM collects and uses your Personal Information as appropriate
to:
• Comply with applicable laws and regulations
• Recruit and evaluate applicants and candidates for employment
• Conduct background checks
• Engage clients or customers for employment opportunities for job
applicants and candidates
• Manage candidate or employee employment relationships with Maxim,
including for:

• employee training and development;
• the creation, maintenance, and security of your online employee

• reaching your emergency contacts when needed, such as when
you are not reachable or are injured or ill;
• workers’ compensation claims management;

While we do not sell Personal Information for monetary value, we may disclose
Personal Information in such a way that may be considered a sale of Personal

including the categories of Personal Information, the categories of sources
from which we collected Personal Information, the business or commercial
purpose for collecting, selling or sharing Personal Information (if applicable),
the categories of third parties to whom we disclose Personal Information (if
applicable), and the specific pieces of Personal Information we collected about
you;
(2) The right to delete Personal Information that we collected from you,
subject to certain exceptions;
(3) The right to correct inaccurate Personal Information that we maintain
about you;
(4) If we sell or share Personal Information, the right to opt out of the sale or
sharing;
(5) If we use or disclose sensitive Personal Information for purposes other than
those allowed by the CCPA and its regulations, the right to limit our use or
disclosure; and
(6) The right not to receive discriminatory treatment by CHM for the exercise
of privacy rights the CCPA confers.
How to Submit a Request to Know, Delete, and/or Correct

If you are submitting a request on behalf of a California resident, please submit
the request through one of the designated methods discussed above. After
submitting the request, we will require additional information to verify your
authority to act on behalf of the California resident.
Our Process for Verifying a Request to Know, Delete, and/or Correct
We will comply with your request upon verification of your identity and, to the
extent applicable, the identity of the California resident on whose behalf you
are making such request. We will verify your identity either to a “reasonable
degree of certainty” or a “reasonably high degree of certainty” depending on
the sensitivity of the Personal Information and the risk of harm to you by
unauthorized disclosure, deletion, or correction as applicable.
For requests to access categories of Personal Information and for requests to
delete or correct Personal Information that is not sensitive and does not pose a
risk of harm by unauthorized deletion or correction, we will verify your identity
to a “reasonable degree of certainty” by verifying at least two data points that
you previously provided to us and which we have determined to be reliable for
the purpose of verifying identities.
For requests to access specific pieces of Personal Information or for requests to
delete or correct Personal Information that is sensitive and poses a risk of harm
by unauthorized deletion or correction, we will verify your identity to a
“reasonably high degree of certainty” by verifying at least three pieces of
Personal Information you previously provided to us and which we have
determined to be reliable for the purpose of verifying identities. In addition,
you must submit a signed declaration under penalty of perjury stating that you
are the individual whose Personal Information is being requested.
Right to Opt Out of Sale or Sharing of Personal Information
If you are a California resident, you have the right to direct us to stop selling or
sharing your Personal Information. You may submit a request to

SHINE THE LIGHT LAW
We do not disclose personal information obtained through our Sites or Services
to third parties for their direct marketing purposes. Accordingly, we have no
obligations under California Civil Code § 1798.83.
